(טז) וְהָיָ֤ה לְאוֹת֙ עַל־יָ֣דְכָ֔ה וּלְטוֹטָפֹ֖ת בֵּ֣ין עֵינֶ֑יךָ כִּ֚י בְּחֹ֣זֶק יָ֔ד הוֹצִיאָ֥נוּ יְהוָ֖ה מִמִּצְרָֽיִם׃ (ס)
(ה) יְכַוֵּן בַּהֲנָחָתָם שֶׁצִּוָּנוּ הָקָּבָּ''ה לְהָנִיחַ אַרְבַּע פָּרָשִׁיּוֹת אֵלּוּ, שֶׁיֵּשׁ בָּהֶם יִחוּד שְׁמוֹ וִיצִיאַת מִצְרַיִם עַל הַזְּרוֹעַ כְּנֶגֶד הַלֵּב, וְעַל הָרֹאשׁ כְּנֶגֶד הַמֹּחַ, כְּדֵי שֶׁנִּזְכֹּר נִסִים וְנִפְלָאוֹת שֶׁעָשָׂה עִמָּנוּ, שֶׁהֵם מוֹרִים עַל יִחוּדוֹ וַאֲשֶׁר לוֹ הַכֹּחַ וְהַמֶּמְשָׁלָה בָּעֶלְיוֹנִים וּבַתַּחְתּוֹנִים לַעֲשׂוֹת בָּהֶם כִּרְצוֹנוֹ, וִישַׁעְבֵּד לְהָקָּבָּ''ה הַנְּשָׁמָה שֶׁהִיא בַּמֹּחַ וְגַם הַלֵּב שֶׁהוּא עִקַּר הַתַּאֲווֹת וְהַמַּחְשָׁבוֹת, וּבָזֶה יִזְכֹּר הַבּוֹרֵא וְיַמְעִיט הֲנָאוֹתָיו וְיָנִיחַ שֶׁל יָד תְּחִלָּה וִיבָרֵךְ לְהָנִיחַ תְּפִלִּין, וְאַחַר כָּךְ יָנִיחַ שֶׁל רֹאשׁ וְלֹא יְבָרֵךְ כִּי אִם בְּרָכָה אַחַת לִשְׁתֵּיהֶם: הַגָּה: וְיֵשׁ אוֹמְרִים לְבָרֵךְ עַל שֶׁל רֹאשׁ עַל מִצְוַת תְּפִלִּין אֲפִלּוּ לֹא הִפְסִיק בֵּינְתַיִם (הָרֹא''שׁ הל' תְּפִלִּין) וְכֵן פָּשַׁט הַמִּנְהָג בִּבְנֵי אַשְׁכְּנַז שֶׁמְּבָרְכִין ב' בְּרָכוֹת וְטוֹב לוֹמַר תָּמִיד אַחַר הַבְּרָכָה הַשְּׁנִיָּה בָּרוּךְ שֵׁם כְּבוֹד מַלְכוּתוֹ לְעוֹלָם וָעֶד (מַהֲרִ''י בֵּן חָבִיב אָגוּר סי' ל''ח).
(5) When putting them on, have in mind that God commanded us to "put these four passages which contain [the principle of] monotheism and the Exodus on the arm opposite the heart and the head opposite the brain so that we may remember the miracles and wonders that He did for us which indicate His Unity and that He is omnipotent in heaven and on earth." And to submit to God his soul, which resides in the brain as well as his heart which represents physical desire. Through this he will remember the Creator and moderate is pleasure[-seeking]. He should put on the arm-tefillin first and say the blessing "... to place the tefillin" and afterwards put on the head tefillin without a second blessing. Note: Some say to make a second blessing on the head tefillin "...on the mitzvah of teffillin" and this is the Ashkenazic custom.
(ט) אָסוּר לְהַפְסִיק בְּדִבּוּר בֵּין תְּפִלָּה שֶׁל יָד לִתְפִלָּה שֶׁל רֹאשׁ, וְאִם הִפְסִיק מְבָרֵךְ עַל שֶׁל רֹאשׁ עַל מִצְוַת תְּפִלִּין: הַגָּה: וּלְדִידָן דְּנוֹהֲגִין לְבָרֵךְ ב' בְּרָכוֹת אַף אִם לֹא הִפְסִיק צָרִיךְ לַחֲזֹר וּלְבָרֵךְ עַל שֶׁל רֹאשׁ לְהָנִיחַ וְגַם עַל מִצְוַת (ד''ע).
(9) It is forbidden to speak at all between putting on the two tefillin, and if he spoke, he must make the blessing on the head tefillin. Note: Since we [Ashkenazim] make two blessings, he must make both blessings on the head tefillin
(לא) (לא) ולדידן דנוהגין וכו' - כי הטעם שאנו מברכין שתי ברכות על תפילין ולא די בברכת להניח לבד הוא שאנו סוברין דעיקר תיקון הברכות אלו כך היתה בתחלת הנחתם תקנו לברך להניח וקאי נמי על ש"ר וכשמניח הש"ר ומהדקו מברך נמי על מצות שזו היא גמר המצוה הילכך ממילא אם סח והסיח דעתו צריך לחזור ולברך גם להניח על הש"ר [הרא"ש]:
(יג) נָהֲגוּ הָעוֹלָם שֶׁלֹּא לַחֲלֹץ תְּפִלִּין עַד אַחַר קְדֻשַּׁת וּבָא לְצִיּוֹן. וְיֵשׁ מִי שֶׁכָּתַב עַל צַד הַקַּבָּלָה שֶׁלֹּא לַחֲלֹץ (תְּפִלִּין) עַד שֶׁאָמַר בָּהֶם שָׁלֹשׁ קְדֻּשּׁוֹת וְאַרְבַּע קַדִּישִׁים דְּהַיְנוּ לְאַחַר ק''י וְהָכֵי נוֹהֲגִים הַמְדַקְדְּקִים (פֶּתַח עֵינַיִם ס' הַמּוּסָר פ''ד) וּבְיוֹם שֶׁיֵּשׁ בּוֹ סֵפֶר תּוֹרָה נוֹהֲגִים שֶׁלֹּא לְחָלְצָם עַד שֶׁיַּחֲזִירוּ סֵפֶר תּוֹרָה וְיַנִּיחוּהוּ בַּהֵיכָל: הַגָּה: וְהַיְנוּ בְּמָקוֹם שֶׁמַּכְנִיסִין הַתּוֹרָה לְאַחַר וּבָא לְצִיּוֹן גּוֹאֵל אֲבָל לְפִי מִנְהַג מְדִינוֹת אֵלּוּ שֶׁמַּכְנִיסִים הַתּוֹרָה מִיָּד לְאַחַר הַקְּרִיאָה אֵין לְחָלְצָם רַק כְּמוֹ בִּשְׁאָר יָמִים (דִּבְרֵי עַצְמוֹ) וּבְיוֹם רֹאשׁ חֹדֶשׁ חוֹלְצִים אוֹתָם קֹדֶם תְּפִלַּת מוּסָף: הַגָּה: וְהוּא הַדִּין בְּחֹל הַמּוֹעֵד וְדַוְקָא בְּמָקוֹם שֶׁאוֹמְרִים בְּמוּסָף קְדֻשַּׁת כֶּתֶר. מִיהוּ נוֹהֲגִים לְסַלְּקָם קֹדֶם מוּסָף בְּכָל מָקוֹם (בֵּית יוֹסֵף):
(13) ...The custom of the world is to refrain from removing Tefillin untl the recital of "Kedushat U'ba Lezion." There are some who wrote based on the Kabbala to not remove Tefillin until the three kedushot and the four kaddishim which is after Kaddish Yatom and this is what the particular are accustomed to. On a day where there is the reading of the Sefer Torah, we are accustomed to remove them after the Sefer Torah is placed in the Hechal. Rama: This refers to a place where the Sefer Torah is returned after U'ba Lezion, however, according to the custom of our province where we return the Sefer Torah right away , we should not remove it like similar days. On Rosh Hodesh, we remove the Tefillin before Musaf. Rama: This applies to Hol Hamoed and specifically when we say musaf kedushat keter. However, we are accustomed to remove them before musaf.
(ו) וְאִטֵּר יַד יְמִינוֹ, אִם עוֹשֶׂה כָּל מְלַאכְתּוֹ בִּשְׂמֹאלוֹ מֵנִיחַ בִּשְׂמֹאלוֹ, שֶׁהוּא יָמִין שֶׁל כָּל אָדָם, וְאִם שׁוֹלֵט בִּשְׁתֵּי יָדָיו מֵנִיחַ בִּשְׂמֹאל כָּל אָדָם, וְאִם כּוֹתֵב בִּימִינוֹ וּשְׁאָר כָּל מַעֲשָׂיו עוֹשֶׂה בִּשְׂמֹאלוֹ, אוֹ כּוֹתֵב בִּשְׂמֹאל וּשְׁאָר כָּל מַעֲשָׂיו עוֹשֶׂה בְּיָמִין, יֵשׁ אוֹמְרִים שֶׁיָּנִיחַ תְּפִלִּין בַּיָּד שֶׁתַּשׁ כֹּחַ דְּבָעֵינָן יָד כֵּהָה, וְיֵשׁ אוֹמְרִים שֶׁהַיָּד שֶׁכּוֹתֵב בָּהּ הִיא חֲשִׁיבָה יָמִין לְעִנְיָן זֶה, וּמֵנִיחַ תְּפִלִּין בַּיָּד שֶׁכְּנֶגְדָּהּ: הַגָּה: הָכֵי נָהוּג.
(6) A lefty who does all his work with his left hand puts his Tefillin on his "left hand" which is the right hand of all people. And if he uses both hands, he places them on the left of all people. If he writes with his right hand and does all otherr work with his right hand, there are those who say to put the Tefillin on his weaker hand because we "need the weaker hand." And there are those that say that the hand with which he writes is considered the right hand for this and he puts his Tefillin on the other hand.
(כב) (כב) יד ימינו - ואפילו נעשה איטר ע"י שהרגיל עצמו אח"כ ולא נולד כך יניח בימין וכ"ש אם מן השמים הרגילוהו דהיינו שנולד לו חולי בימינו וניטל הכח ממנו או שנקטע לו כף ידו הימנית וצריך לעשות כל מלאכתו בשמאל בודאי דינו כאיטר גמור ויניח על היד הרצוצה ואם חזר לבריאותו ונעשה שולט בשתי ידיו בשוה הרי הוא ככל אדם:
