Talmud Pesachim, 117b

R. Shimon b. Lakish said: [G-d said to Abraham] “I will make you into a great nation”-- this is what we recite in the Amidah prayer ‘G-d of Abraham.’ “And I will bless you” – this is what we recite ‘G-d of Isaac.’ “And I will make your name great”—this is what we recite ‘G-d of Jacob.’

You may think that we conclude the blessing [on the Amidah] with all three of them, so the verse continues “And you will be a blessing,” in you we will conclude and not with all of them.
